Welcome to our Chapter Delegates and thank you for joining together in Ottawa, Ontario for our annual symposium. Registration ends 6pm Mon, Sept 26, 2016.
The registration fee of $225 includes the full 2 1/2 day sessions, receptions, Friday and Saturday lunches and the Friday evening Awards Dinner. Registration can be completed using one of the following methods:

2. Register By Mail (payment by cheque or credit card). Click on link below to download registration form
3. Register by Interac E-transfer. Send the $225 registration payment as an e-transfer through your online banking. The payment is directed to our address: accounting@CW4WAfghan.ca. Then also email that same address with your name, address, chapter location and any special food requirements along with the required security answer (there may be a small fee charged by your bank for this transaction.)
